I plan to write something similar to this:

_____ Will Attend _____ Will Not Attend
And I'm going to put the RSVP date a month an a half before the wedding.

I have to have my out of town guests respond about 2 months before the wedding because the hotel block rsvp date is 1 month before the wedding.

The favour of a Reply is Requested by the first of April M__________ Accepts___ Declines___ ___ of ____ attending
(fill in the last blank with the number of people you are inviting, like 5 if you are inviting a family, but then they fill in 2 if only 2 are attending.)
