Go to the town you were born in with birth certificate and ID, and I think thats it??? Or the town hall where you currently live.

I just got my marriage license and although I live in Queens, I went out to Huntington because we're getting married in the church out there, and we needed to drop off paperwork with the church at the same time. There was no problem, and all you need is your Driver's license or passport, your birth certificate, and if either of you were previously married, your divorce certificate.
