Marriage License forms

Going for marriage license tomorrow...is there a form I can print out at home and have it filled out before we go? just curious if anyone did that ....thanks!

I'm not 100% sure, but I think you can only get the forms online if you are going to get your license in the City (Queens, Brooklyn, NYC, etc.) I don't think Nassau and Suffolk have the forms online.

we're going to get our license tomorrow at the ToH. If they give me an extra copy, I can scan it and email it to you..where are you thinking of getting your license?

thanks girls!! i appreciate your help. we went this morning to the islip town hall, and it took 15 minutes.... it was a breeze!

Just want to add....I had my hospital birth certificate and not my city birth certificate ....but they still accepted it. They also gave me paperwork on how to obtain the a copy of the city one.
