The Knot has a whole section on this I think.

Off the top of my head though:

Married couples:
Mr. and Mrs. John Smith

Singles:
Mr. John Smith
Ms. Jane Smith

Doctors:
Dr. & Mrs. John Smith
Dr. John Smith (if single)
Dr. Jane Smith & Mr. John Smith (if wife doctor)

Is it true that your supposed to address a married couple using the Mr. and Mrs. Husband's first name, Couple;s married name? What about the wife's first name?

Nope traditionally the wifes first name is left out so it would be

Mr. and Mrs. John Smith

But maybe you can find a sight that is more modern and will put both their first names, dont know i didnt see it

Yes, you're supposed to use the husbands first name and their married name then on the inside envelope, if you're having one, you can use the wife's first name.

Also, if they have different last names you can address as follows:

Mr. John Smith & Mrs. Jane Doe
