For question 1, I did mine as your choice a&c. I put Ms. Ann & Mr. Joe if Ann was my primary guest, or I would do Mr. Joe & Ms. Ann if Joe was the primary guest. Make sense?

For # 2, I did a.

For #3, The caligrapher did choice A, and I was fine with that.

I've read you're only supposed to use 'and' if the couple is married.

so'

unmarried couple:

Mr. John Smith
Ms. Jane Doe
123 Sesame Street

married couple but Jane kept her name:

Mr. John Smith
and Ms. Jane Doe
123 Sesame Street

married couple but Jane changed her name

Mr. & Mrs. John Smith
123 Sesame Street

John is invited but you don't know who he is bringing:

Mr. John Smith and Guest
123 Sesame Street
