RSVP Wording

I am having an issue with the RSVP wording...what do you think of this...

The favour of a reply is requested by the fifth of July.

M _____________________
____Accepts with pleasure
____ of ____ will be attending

____Declines with regret


is the ___ of ___ will be attending tacky? I plan on filling out the second ___ just because I know we have friends that are single and some may not come with spouses etc.

We are doing two types ... but ours will read:

The favour of a reply is requested by the twenty-eighth of April.

M________________________
___ Graciously accepts
___ Regretfully declines

OR

We have reserved 2 seats in your honour.

M_________________________
___ Graciously accepts
___ Regretfully declines
