Small Invitation Details...

We're getting ready to send out our invitations and we've run into some small invitation snafus that we of course didn't think of until now.

1. How does the inner envelope get addressed? Mr. and Mrs. Smith, Mr. and Mrs. John Smith or John and Mary Smith? (I'm getting varying anwers on this one.)

2. Is there a particular order for all the enclosures? ie. direction cards, response and envelope

3. What is the proper wording for a married couple when the woman still goes by her maiden name?

If anyone can think of any other little things I may run into, please feel free to add!
