I sent mine out a month ago- Everyone who was invited already knew when the wedding way, and I couldnt resist!! :)

I just ordered mine yesterday. We have a lot of OOT guests, so I'm sending them out a little early in order for everyone to make flight/hotel arrangements. I think if you have OOT guests, it's better to send them out early.

No official rule. Gewneral rule of thumb is between 6-12 months before with each situation dependent on:
-# OOT guests
-if it is a popular weekend (like aholiday)
We are going to send ours after holiday season -- afraid they might get lost with all of the holiday stuff. Plus this way we had time to collect names and addresses, etc.
