Tipping for Dummies

OK...I know there are a million posts on tipping but let's get it all in one place.

SO far I have:

Maitre D ($2.00 -$3.00 pp at the wedding. Seems outrageous to me but if that's waht's appropriate then OK)
Photographer - $25.00 (seems low)
Videographer ($25.00)
Hair (20%)
Make-Up (20%)
Attendant (?)
Clergy ($50.00 for what I don't know)
Florist Delivery people ($10.00 each)
Band ($20 each)

I don't have limos so that's why I didn't add that. Am I missing anything?

I have a question. How can you tip the photographer when you do not know how the photos will come out? Also, do you include that you are paying for their meals at the wedding and is that included as part of the tip?
