So I didn't really think it would happen even though I read many people post about it on here. But anyway, today I got my first creative RSVP where someone added a guest when only they were invited. What the hell do I do now?

Just wait and see if you get enough No's that it doesn't matter. I had one friend reply with a guest when I hadn't mentioned she could (it's fine -- we're well under our estimate) and the other day she told me she no longer has a guest to bring.
You've got time to feel it out without worrying.

I would give them a call and politely explain that they were invited solo due to the number of guests you have to invite.
