Social Security Card?

Did anyone mail away for their new social security card? I really don't have time to go there. I'm taking off on Friday and was going to go but if I can avoid that it would be GREAT.

I know I would have to mail my original card and hard copy of my marriage license but do I need to send a photo ID with it? I'm going to Italy in July so I need my passport and I can't send my license because I drive to work every day. I guess I could do it after I go to Italy but I wanted to get it all done. Also, do I have to send a check with it too?

One more thing and this is a stupid question probably. When I change my social security card, does that mean it's changed with social security (as in the retirement social security)? Or do I have to send them something too?
