Passport help!

I am filling out the passport renewal form and one of the questions is the place and date of my name change.... dumb question but what do i put here?!!!

marriage date and location?

date and location of marriage lisence?

HELP!

I put down my wedding date and town where I got married.

ETA- actually I just checked and they said its in the final stage of processing. I only sent it last week too!!

ha this is the one question i left blank bc i got the certificate in queens but got married in huntington.... im pretty sure they can figure it out from the certificate..

I'm so mad that you have send your existing passport back to them - I have our HM stamps from Italy in my current book :-\

they send back your original passport ... mine came back with two holes punched in it and one of the new photos stapled to it ... you'll still have it, just can't use it! ...
