How to change my name?

Can anyone give me the steps I need to take?? Thanks so much

I'm looking into this as well, so far I found this website helpful:

1. Obtain certified copy of Marriage License (original). I plan to request at least 3 to 5 certified copies in order to have originals for everything.

2. go to Social security office to change name.

3. DMV office to get new driver license (can also do by mail)

4 Passport (can do by mail, the form is DS-82 and the fee is $140)

5. Bank statements, W-2, W-4 forms at work, credit cards, etc...
