Repost from 05 board-HELP Can someone send me a 6 page program template (Bi fold )

I am having the hardest time trying to find a nice 6 page (incl cover and backing w/ 4 printed pages in between) program template.
Can anyone help me please? and I may be silly but I cant figure out how to print & fold the pages correctly
