names on table cards - repost from 05

If someone is coming with a guest (not married), do you put 'Mr. John Smith and Guest' or 'Mr. John Smith and Ms. Jane Doe' ? And if the two people are the same gender, do you put them on the same card (Ms. Susan Smith and Ms. Jane Doe or a separate card for each or Ms. Susan Smith and Guest). Thanks so much....
