WEDDING DAY TIPPING!

Okay... I know it's early, but I want to figure all this into my budget. Who gets tipped and how much? Does it matter if prices 'include' gratuity? Are you still supposed to tip on top of that? Who and how much for the following:

Priest
Altar servants
Limo Drivers (each one?)
Video
Photography
Maitre D (or however you spell it)
Waiters/Waitresses/Bartenders at reception
Florist
Hair
Make Up

I'm sure I'm missing stuff...
any opinions?

Thanks for bringing this thread back up!

I was talking with my FH about it when I read it the first time and he kind of dismissed me but, after his friend's wedding last month he asked me, did you know we have to tip the band??? Um, yeah, I told you 2 months ago.
