Need letter help!!

I am finally going on hubby health insurance in order to get a raise that I have been waiting for , for 3 years now!!!!

Well Dh told me I need to have my boss write a letter stating when my health insurance policy will no longer be effective and the date.. My boss wants me to write and and she will sign but I am having writers block.. can anyone help with wording?
