Does anyone have a template they used to make the vellum?

I got the candle holders from michael's. I'm going to start playing around right now!

we did ours a little different but it was easier in the long run...we sponge painted the votives white (so they looked almost frosted!) and them used cardstock to create a name plate which we punched two holes in (one on each side - left and right!) and then attached with ribbon in one of our colors....sounds bad but it came out nice! we don't have any pics of it except in our video though so if you do it, make sure to get a physical picture of it!!!
