Looking for a Photo/Video - suggestions or prices

I'm looking for information for both or one items. I don't mind using a different company for both. I've outlined what I'm basically looking for. I'm hoping to spend around $2K max for video and $3K max for photo. Anyone with suggestions in that price range I would appreciate your feedback! Thanks a million.

Photo:
I want a photographer for pictures at my parents house in Western Suffolk and to cover all or at least most of my reception.
- One album for us (I like storybook ones, but am open and senstive to price due to budget)
- Two albums for the parents
- One 8 x 10
- Five 5 x 7
- wallets at least 100
- Thanks you cards (if in package, not necessity)
- Engagement photo
- Digital Prefered, but not mandatory
- CD with proofs
- ALL proofs
- a place online for people to order their pictures (we have more than 100 Out of town guests)
I would like table shots and I'm interested in photojournalism.

Videography:
Must cover the entire wedding from ceremony through the end of the reception. I may want them to cover me getting ready at my parents house (once again depends on pricing)
- No interviews
- DVD prefered (3 copies)
- Chapters
- Bloopers section
- Still shots (depends on pricing)
- Songs
- Baby and other pictures
- Edited
- non intrusive camera operator

i'm not sure about online ordering but it looks like they can do everything else you want

Duet has a really great package deal
you get 2 photographers for 12 hours and a videographer
their work is amazing !!!!!!!

we are using De Ja Vu and it is costing us just about $4500 for both.

Has any one worked with North Shore Video? I met Mike last night and we thought he was great.
