Forms for changing name on passport

I just had to do this and thought I'd pass it along. If you have a current, valid passport, changing to your marital name is really easy. Pop the original passport, new photos ($6 at Genovese) your original marriage certificate (the municipal one, not one from the church) + a form called DSP-19 and put it in the mail. That's right - you do not have to appear in person and its free! Here's the link to download the forms. You'll need Adobe Reader. Its a simple 1 page form and the instructions are on page 2. http://travel.state.gov/get_forms.html

Forms for changing name on passport

I just did it..and just got a call from the passpart office asking if I sent my passport with the form and lic.....it seems they have lost my passport. ( I am a bit worried it was stolen). Also where did you read to send new pictures? From what I found out that wasn't needed, an amendement would be made to my old passport. Did you get it back yet?

The mailing addresses to the various agencies that will process your app are on the second page. I only mailed it mid-February and it takes 4-6 weeks to process. Hope this helps.

I am pretty sure you don't need to send new pictures. On the directions and list of what to send it doesn't mention pictures for a name change. I just hope they found my passport!!!
